Members can log in to the MemberDirect portal at any time and from any device type to retrieve and submit forms online. Members can also access printable forms to get assistance with the various life and career changes.

Active Member Forms

These forms are intended for use by active, terminated, vested, and proportionate service members.

Retiree Forms

These forms are intended for use by retired members, beneficiaries, and survivors.
