What do I need to do to designate a beneficiary to receive my funds in COA ERS in case I die before I retire from COA ERS?
You should complete a COA ERS “Beneficiary Designation Form”. The form (along with important instructions for completion) is available online at our website or by calling the Retirement Office. If you do not have a valid form on file, your death benefits will be paid to your spouse if you are married, or to your estate if you are not married. This important document must be carefully and fully completed and on file with COA ERS.
